This is an in-scope position reporting to the Area/Care Manager on the Team: Integrated Home Care Program, the HCA Supervisor works as a member of an inter-professional health care team. The HCA Supervisor primarily responsible for the supervision of HCAs (Health Care Aids), is responsible and accountable for their own practice according to the standards defined by the College of Licensed Practical Nurses of Alberta. The HCA Supervisor is responsible to assess, plan, monitor and evaluate in consultation with the RN Case Manager- Short Team & Long Term, the care needs of Home Care clients and their caregivers/families requiring community-based services. The HCA Supervisor may also work in multiple settings such as residential care, assisted living, homecare clinics and in the community. Supervise HCAs (Health Care Aids). Assessing, planning, organizing and evaluating the needs and goals for clients and their caregivers/families for acute or rehabilitative client groups. Performing short-term assessments and provide direct nursing care on a consultative basis. As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).
Completion of Continuing Care Case Management. Competency Framework you demonstrate ability in the areas of personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships. Successful completion of a recognized Practical Nursing educational program (Diploma or Certificate in Practical Nursing). Completion of a Medication Administration Course for LPNs. 2 years of work experience as a Licensed Practical Nurse and experience supervising a team. A keen interest in wound care and a willingness to acquire expertise through education & training. Excellent interpersonal, communication and organizational skills, and is a team player. Physically able to perform work related tasks and duties. Reviewing client care needs and goals by planning, organizing and evaluating changes to the care plan as required. Collaborating and consulting with the case managers and or Registered Nurse and the inter-professional health care team as required. Conducting home visits. Identifying clients at risk and providing appropriate nursing intervention or advocacy as required. The successful candidate may be assigned to provide direct nursing care to clients in a client’s home.
This position supports the Diamond Valley Home Care located 64 km south outside of the Calgary City Limits and requires travel to multiple sites. Experience working in the Community a definite asset.
