QUALIFICATIONS
* Health Care Aide Certificate from a recognized Health Care Aide program
* Other combinations of suitable education and experience may be considered
* Province of Manitoba Class 5 Drivers License, or equivalent from province of residence, and access to a personal vehicle to provide service within Prairie Mountain Health
* Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology Demonstrated organizational skills, and the ability to work independently
* Demonstrated problem solving and decision-making skills
* Demonstrated flexibility to facilitate changes in techniques and procedures in a changing environment
* Demonstrated knowledge and competence of skills and concepts related to the position
* Demonstrated communication skills
* Ability to respect and promote confidentiality

POSITION SUMMARY:

The Home Care Attendant provides direct client care and environmental support within established standards of care, policies and procedures. The Home Care Attendant functions to meet the needs of clients, assisting them in maintaining optimum health and independence for as long as safely possible in their home environment.

RESPONSIBILITIES:

Overview:

  • Demonstrates respect for client independence, dignity, privacy, and lifestyle at all times.
  • Supports clients’ independence in activities of daily living, as assessed by the Case Coordinator.
  • Demonstrates knowledge of standards for safety and sanitation including guidelines for hand washing. Reports concerns of knowledge and/or skill deficits to the Supervisor.
  • With additional education and in accordance with PPG-00819, Assignment of Medication and Treatment Tasks to Home Care Attendants, may be assigned additional tasks as required.
  • Demonstrates ability to effectively utilize electronic programs.
  • Performs all communication functions (mail, email, photocopying, faxing, telephone) as required in accordance with Personal Health Information Act (PHIA) and Privacy Act.
  • Assists with meal preparation in accordance with the “Canada Food Guide” or the client specific diet.
  • Organizes work according to client’s needs and the assigned service request.
  • Reports any changes in the client status or home environment to the Supervisor and/or Case Coordinator as per PPG-00738, Observation Reporting- Home Care Program and PPG-02154, Observation Reporting- Supportive Housing Program
  • Reports incidents immediately to the Supervisor or Case Coordinator and/or the client contact and enters in the Incident Reporting System as required.
  • Participates in staff meetings, case conferences, and in-service programs as required. Communicates with co-workers involved to achieve the client-centered goals.
